If you would like to be considered for an upcoming user research session, please fill out the form below and click the Submit button.

Required fields are preceded by an asterix (*). The other fields are optional; however, the optional fields are often used as the basis for selecting participants for a particular test session. If you prefer to receive a form that you can print out and fax in, rather than completing this form online, then fill out the fields with asterisks in the form below, and in the Comments field type in the words "Send form." Note that all test sessions are videotaped, so if you do not want to be videotaped, please do not sign up.
